FIRST APPOINTMENT & CONSULTATION* (Required for New Patients)
Your initial appointment with Skin365 is the first, and most important, step towards reaching your cosmetic goals. At your first appointment, you will receive a one-hour consultation with one of our highly-trained practitioners. They will spend the time to learn about what you would like to achieve with non-invasive cosmetic treatments, discuss any issues or concerns you may have, and complete a comprehensive evaluation of your skin. With this information, a customized treatment plan will be developed that addresses your aesthetic goals and provides for the most effective results.

* A $100 non-refundable deposit will be collected at the time you make your first appointment with Skin365. Deposit includes: one-hour consultation with a Skin365 practitioner, comprehensive skin evaluation, individualized treatment plan. The $100 deposit will be applied towards a cosmetic or skincare treatment at your first appointment.

Cancellation/No Show: If you cancel or reschedule your first appointment less than 24 hours prior to your appointment time, the $100 deposit will be charged as a “no show” fee and will not be applied to your first appointment/consult.

Please print out, complete, and bring the attached forms with you to your first appointment. Please arrive 10 – 15 minutes before your scheduled appointment time so we may review your paperwork prior to meeting with you.
